No need to submit any IDs or doctor prescription, but you will be asked to sign a form declaring that you are undergoing for this test willfully.
If you are in a short window period (like 12 days) then go for PCR RNA. But it is not done everywhere and it requires doctors' prescription and it may take 3 days for the result. It costs around Rs. 4000/ Some hospitals like citizen collects the samples and sends them to microbiology centers that carry out these tests.
If you are less than 3 days from a possible exposure then take PEP from a known doctor or request some reliable doctor by offering some money.
