Regular Grabs are often tiny sub compacts, even smaller than the regular white taxis. However, the app does have the option to book a 'XL' car which will cost slightly more, have less available but will be far more comfortable.
If you really struggle with mobility, it wouldn't hurt to keep the WhatsApp number of preferred drivers / cars and call them direct.
If I was a smart ass, I'd suggest that walking around Manila or AC in the midday heat will soon see you down at 250. 😜.
With a little preplanning you will be fine.

I am bigger and taller than you and can answer based on my experience. My experience is in Angeles City not Manila. But:
If able to get a Grab. Get a Grab! The lady will make herself fit in the Trike. She can get on the side. Like the fender and hold on to you. There are some larger trikes and you can request. But the trikes have un-holy Suspension and the potholes will fuck your back up. Ask me how I know this. Trikes can be wide enough and you and her can make it work. But the Bumpiness with our weight just makes it uncomfortable. So get a grab car if available. And again, don't worry about her. Worst case you get in on trike an she gets in another go to the same places. But yeah, very uncomfortable. At our size.

I've been researching this for a long time here, on YouTube and other blogs / vlogs and I have yet to get an authoritative answer. I'll appreciate anyone here that provide accurate guidance.
I am big guy: 6'1" tall, weigh about 320 lbs /145 kg, and I'm 61 years old. My health is ok, not great, and I have difficulty walking more than a few blocks.
My concern is about being able to use trikes to get around. Will I fit comfortably? If I want to bring a bar girl to a hotel (mine or a sogo) will she fit in a trike with me comfortably? It's really hard to know by just reviewing videos from the area.
If the answers are no, is my next best option booking a Grab through the mobile App? I realize that on Walking Street in Angeles, getting a Grab during peak hours means moving to a nearby, parallel street. That seems doable.
Finally, if I use Grab am I better off paying through the App or should I wait to negotiate with the driver and pay cash.
Thanks again for any insights you can provide.

One thing that catches people by surprise is yellow fever. Entering Philippines from say, Brazil, or Africa youd need Proof of yellow fever shot. But I dont think Thailand is on that list, better double check.
Regarding hotels and stuff, its no longer required by law but that dont mean the person working there is up on it. And the individual establishment has the right to enforce whatever policy they choose, I suppose. Its their place. WC mentioned that someone hit him up, and his FUBU wasnt allowed entry. So you better check with the place before you book.

I'm planning to fly to the Philippines next week from Thailand for a change of scenery and I have a couple of questions if I may:
For those that are not fully vaxxed, what is the requirements to enter the Philippines? I read on Kayak, that the unvaxxed can enter the Philippines with a negative COVID-19 test result.
Is that it?
And are the hotels / airbnbs still requiring vax card to register / check in and enter each time?
I'm mostly looking at Manila and Cebu at this time.
Appreciate all the feedback guys.

It's unclear why you want to keep a strict account of your spending. If it's just preplanning, then, unless you run wild with drinks, your budget is generous. If you want to limit spending, then just spend more time in AC than MNL and make use of cheaper FL and online options.

I suppose Filipinas will be changing SIM's even faster now than before. They only cost about 50 pesos at about every sari sari store. Probably just a matter of time before it's like most countries where you have to bring your document into the store and register before you can even get one.
