You are much better informed than Moi on current conditions and on what you specifically like, but if I were in your position, I would skip option #1 BKK and option #4 Chiang Mai for sure. BKK is going into another lockdown and it has no beach so forgetaboutit. Chiang Mai is a BYO kind of place especially now so forgetaboutit too.

That leaves you with option #2 & option #3 both of which sound kind of good to me. Option #2 provided you can find a one way car rental, would have a lot of sightseeing plus you can take your time and stop off in Hua Hin on the way to Pattaya. Option #3 sounds good too ie just keep doing what you are doing and have a good time until the day before your scheduled flight back home when it will be time to get up to BKK.

With the Covid measures in place Pattaya probably is only marginally better than Phuket girly wise, but Phuket has many beautiful beaches which at the present time are left almost all to yourself which is a once in a lifetime event. I know because I was in Rio de Janeiro in April during their lockdown and all the beautiful RJ beaches were semi deserted and all mine. It was amazing.

Remember that in Pattaya you will need to find a hotel with a decent swimming pool. The Beach in Pattaya looks good, but you can't go in the water unless you are out of your friggin' mind. LOL. So, I seem to be leaning towards option #3: staying in Phuket, but option #2 driving up the Kra Isthmus, seeing the sights, stopping off in Hua Hin for a night or 2 or 3, and driving to Pattaya is not bad idea at all.

Context: I arrived in Patong on July 3rd as part of the Sandbox program. So I am giving a synthetic presentation of the 3 days I just spent here, I understood that some were interested in feedback. The stage being set, let's get down to business. The remaining 5% of activity is concentrated in spots. So, in the hotel where I am, there are a few tourists which gives the impression of vacation. Likewise, a few restaurants are open on the beach road. Tourists, expats and also pretty Thaies. And we also serve alcohol. Everything seems (almost) as before in these few places.

Otherwise, between 2 meals, I was able to have dates. First source from Paris: Thayfriendly. Do not be too de-illuzion (connoisseurs will appreciate the spelling), for my part I collect 90% of waste on these dating sites (photos where they were young, thin and beautiful but that was before, financial claims really unreasonable, crappy dates, etc.). But some succeed.

Otherwise, in Patong, a lot of massage parlors are open. It's best to make your choice in situ. A few girls in each living room but it's not the crowd. Enough, however, to find the perfect fit. I would say that even then, it is much better to be in Patong in the event of covid than in Marrakech or Cuba under normal circumstances.

Regarding your post, there have since been two now Farang infections on Phuket, though there are no real details on these. Here is what I posted on Thaiger which might give those considering the trip, something to think about:
