    It is possible he needs surgery. It's also possible, even likely, he doesn't need surgery. Unless you're an orthopedist, who has examined him, you really can't know whether surgery is the best course of action.

    I've been dealing with lower back issues for 25 years. At the beginning, first doctor said surgery was the answer. Second doctor, an Orthopedic surgeon specializing in sports medicine, didn't think so, but wanted to get an opinion from a neurosurgeon.

    The neurosurgeon explained my case like this. I could have surgery and I would have some manageable pain every day, but I would lose some flexibility and mobility. The other option was to finish the physical therapy treatment, take care of myself and my back and most days I would be pain free, but occasionally, if I over exerted, I would have extreme pain.

    I've had a few episodes in 25 years, usually from over exertion or mishaps at work. In most cases, a few days rest fixes the problem. Haven't used any pain medication for years.

    See a doctor or two, get an accurate diagnosis of the problem, then figure out your best path.

    Here is my long 2 cents.

    From personal experience. Over 40 years years ago I came down with my first lower back muscle spasms. Because I was very active and never experience any like this before couldn't even bend down and tie my shoe laces. I did basically a term paper on this subject and what I found I do not think it has change much in 40 years and that is surgery should only be done when you finally hit the end of the road.

    From your last commend, if you haven't even seen a Doctor, had a x-ray, etc. And you are looking or taking medication as strong as what you are looking for that is not a very good sign. Lucky from my work, family member who is a Orthopedic surgeon, another who was a physical therapist with the S. F. Niner's in the 70's I was able to get treated and educated by some of the best Doctors in the country including the Doctor who operated on Joe Montana the Quarterback for the Niner's only after months of elevation and Physical Therapy.

    What I found out in my own case my problem would be with me for the rest of my life but as long as I follow the recommended physical therapy I could manage my pain so I could pretty much could live a normal live without taking any medication. Through the years I had a number of relapses told I might be at the end of the road but whenever I heard the word surgery I work my ass off to stay away from it. The final big blow to my spine was being hit accidently by a malfunction aircraft door latch that cause the door to slam down on my forehead. When I woke up in the hospital still seeing the stars I was told if I was a inch taller I would have been dead. Aside from the conclusion my spine told a big blow it basically crush my spine. Although I had feeling and movements on my lower legs my sciatic was lock blocking off circulation to my left leg. For a week I went through Physical Therapy, saw a Chiropractor, had Acupuncture I did it all everyday to try to unlock the nerve. The Doctors told me I only had a few days left because my leg now was swelling and turning blue. If by days end nothing work, they would put me on traction and if that didn't work surgery. It was also my last day to see the Chiropractor for the last remaining disc, he strapped me in and the machine started to rock and bend me and after a minute I and everyone in the room heard a loud pop, thereafter I felt the blood rushing down to my leg.

    Since that day again I had relapses but nothing like before because I educated myself about my back and how to lift heavy items which was my profession to keep from going under the knife. Through the years I've seen cases and cases of my fellow workers taking bad advise and taking the easy way out and had surgery non of them ever return to work.

    The easy way out is taking medication, just open you month drop it in your mouth and swallow and mask the problem! That is what my cousin does. Today he is only 60, had six operation look like he is 90 year old. He is so bad off, he has tons of money but can't get any pussy. He came two years ago to Pattaya, barely could walk down W. S. Not even wanting to pay 5,000 baht would the stunner fuck him not even a top performer from Soi 6, I ask her why she took a runner you make me look bad? She told me when she got to his room she saw 20 plus medication bottles on his night table which scare the shit out of her. Finally, I arrange a top performer for him at his hotel room and I mean top, very understanding of his problems she work his ass off all night fuck him every which way and gave him more sex in one night he has had his whole life. She left him in one piece at 6:00 am, at 6:30 he called me and said he had to go to the hospital he couldn't move. Luckily knowing he was in bad shape we arrange his hotel to be right next door to the International Hospital on Soi 4, In the end, the girl fuck him so much he had muscle spasms. In the end, more pills no mention of Physical Therapy but that is not surprising in the hospitals in Thailand and Bumrungrad is no different! Since returning home he has has two more operations. He has so many track marks from the operation only the old and hard up for money will go with him if he could come to Pattaya but he can't because he is addicted to the medication and is currently in detox. Now his friends and our family are say I'm right that having a operation is to relief the pain and after if the Doctor is still giving him medication to me something is wrong?

    You can take the easy way out take a pill, but I suggest you go home and see a Orthopedic Surgeon I highly suggest one from Sport medicine, they take the conservative approach and get a full evaluation instead of popping a pill.

    Two years before I left my job, I was ask by my Workmens Compensation department if I wanted to see a Psychiatrist? Never heard of them before I did some research and try to see one at my Sport Medicine Group but they were no openings for two months. I agree to see one recommended by my work. In our first meeting he already had 40 years of history on me. After looking at all the x-rays scans he said to me " I can't fix you but I can help you with your pain so it is more spread out so you would feel better " After two years and each visit didn't seem much he was doing more education than anything and a few exercises after two years of visits I never felt better. The best thing about Thailand are the massages and I've found a person who really knows their stuff and I go 3/4 times a week and that is what has kept the little rabbit hopping!

    Before I end, yes long. See a really doctor at home and forget the pills unless you really need it. Doctors here just want to sell you with a expensive new tech procedure.

    Much appreciated guys, upon my next visit to Bkk I will make a few tests in hospital including a scan, guess there's a hospital that specialize only in back pain issues.

    The good news though is a doctor's appointment is relatively cheap in Bangkok. I used to go to Bumrungrad but now prefer Bangkok Hospital, although it is not as conveniently located. Both are certified by the Joint Commission International.

    For this stuff you need help from doctor. Used this medicine about 6 yrs ago. Did a medical examination at Bumrungrad. They found my real problem and have not used Lyrica ever since.

    Generic name pregabalin. That appears to be some heavy stuff. If it is available then you are sure to need a prescription from a doctor to get it.

    The tapes in CD form in my 7 year old car that has a CD player as original equipment. Thanks to all who tried to help.

    Just searched and you can buy a cassette deck that plugs in to your computer via USB and converts to CDS or MP3's. They're about $100 US. Otherwise I'm sure someone at MBK can easily do this for you for a nominal rate.

    Another good option is a simple radio adapter. Plugs into walkman and then sends out mini radio signal at like 88.1 FM so you just tune your radio and are in business. Used one of these for years as devices change but adapter works for all in any car. Needs separate batteries, but not a big deal.
