It's not like that. Just about all restaurants in Brazil, no matter what kind of restaurant it is, will charge 10% service charge as long as they serve you food or drinks at a table. If you buy your beer at the bar or go to a self-service restaurant there won't be a service charge.
Side-note: Sometimes the bar or restaurant have musicians playing and often they will charge you a couvert fee because of the music. This fee is optional and there is no problem asking them to take away that fee from your bill.
I can swear (but not on my life) that I didn't pay 10% in some hole-in-the-wall places in Rio. I guess, I'll pay attention when I visit next time.
As to couvert, they also charge it for bread, olives and other small munches. Not much, but I usually tell them to take it away. Except when they bring pao de cueijo. Love that shit.

Flights between Rio and SP are tricky. One day it's 30 bucks, next day -- 300. Whatever their computer tells them.
There are three Brazilian carriers: LATAM, Azul and GOL. Some folks prefer one over another, I personally don't care. To me, they all offer nearly identical service.
If you're using Google Flights, try the following combinations.

I got it a few times. There were two times that I remembered distinctly. One I remember I think is at Coco Bambu. The restaurant is large and I was far away with my view blocked and I was charged a fee. The other time is in a bar that it was clearly marked on the table. The waiter also came to tell me about the charge on that table and told me there were other tables that were free.

I asked the receptionist, they said that the maid will get in trouble. They can only take it if there was a written note or if I was there to say it is OK. The standardization is why I love the Mercure in Brazil, however a little expensive compare to other hotel. Never once, did any one give me any issue about bring girl, Mercure in Sao Paulo or Rio.
