I have stayed there many times and never booked for two people.

It has never been a problem either.

Most times there has never been anyone at reception to take her ID card, so I have walked straight to the lifts and the room.

If you want it taken, there is always someone in the back office behind reception who can come out and do so.

That depends. If the hotel offers breakfast and you include that in your booking, you will pay more for 2 persons. Even when it's only you who's having breakfast.
