It is levied by the airport (lots of airports try to make ancillary revenues by milking passengers in many ways). It has been authorized 3-4 years ago by the Department of Civil Aviation (ruling airport areas). The weird thing is that the authority didn't make it compulsory to be paid (not cashed by them, so they don't care) but it is up to the taxi driver. Pretty sure drvers have to pay it to the airport handler AOT.
Taking a taxi deommdepeture level is officially prohibited and police patrols the lanes enforcing the law from time to time, but often not.
Can be good if and when lines are massive at arrivals (less than before). Still taxi drivers try to scam with exaggerated fixed fares. Just be assertive on using the meter plus highway fees. Or set a deal sign you, 400 b. Aware more than enough to go to sukhumvit incl tolls (the two station route).

In SVB the ticket from the machine shows the taxi bay number, the taxi reg number and the driver's name. He will be waiting for you. They obviously punch in their details when they enter the taxi bay.
I did not see any add-on charges on the ticket.
There is an officer standing at the machine to direct people to their waiting cabs.
Even if there is a line it moves remarkably fast.
I have never needed to waste time traipsing up to "Departures" with baggage and then trying to catch a waiting cab who has dropped off departing passengers.
For me, the system at Arrivals works well; I have never had any hassles or drivers trying anything to scam me.

Anyway due to competition can ask any driver his price and simply move to next one if offer is not good. I suppose it's much easier for me speaking Thai fluently. They will know there is no point ro try to rip me off.
To me, any ridiculous price is simply an indicator they do not really want my business. In other places, parked taxis never want my business. I catch moving taxis passing by, stopping them to talk to them of course.
