You have to take control and make it all happen yourself, which can be a good thing or a bad thing depending on your preferences. But if you want to have a good time with a thai girl, you have to really be the man and take her.

Quality of Sex & Girls:
Poor! Most girls want to fuck in the dark, don't feel well without covering themselves with a towel etc. Exceptions of course exist.
I'd say I have a 95-99% rate of girls who love to fuck with the lights on and uninhibited and love being naked, the shy towel girl with lights off is the exception, and usually involves the ones with stretch marks from a kid which I usually find out about before we go home.
But I prefer that a girl likes what she does (even though she primarily does it for money) and this is hard to find here and I found several girls crying. Conversations are also extremely hard. All of them will ask you the same stupid small talk questions (where are you from, how many times in Thailand, do you speak Thai (this can reduce the prices, seems that many have replaced the "how many times you visited Thailand" with "do you speak Thai"). They are all interested in learning English, but if it's not your level to teach a ***** basic English simply forget it. The cultural difference (Buddhist rice-farmer versus you) is too high in my view (of course, this is subjective). If I compare it to South America where I could talk with most of the girls the whole night even though we did not speak the same language at all, then Thailand is simply poor. My opinion!
Again, choose carefully. Yeah, world over most working girls probably don't "like"what they do, but I'd say most of the time here the sex is fantastic, the attitude great, and the percentage of repeats high. Again, choice is everything. Your comment on conversation is probably spot on, then again, you have to remember that Thai is a fairly far removed language from English, especially compared to Spanish, and yeah, if the girl is uneducated and not working that long, well of course she cannot speak English. I do have friends here who cannot speak Thai who seem to be able to "talk" to the girls all night without the same language (although I wonder about comments like this). I speak both Thai and Spanish, and would have to say that I have had far better conversations in Thailand than I ever had in my mongering in S. America.
But as Thais are very prudent on sex, I would assume that the still high HIV rate in Thailand mostly comes from Prostitutes. Believe it or not, at every level of Thai society, there is a major lack of prudence about sex. If a girl sees you in any way, shape, or form as a boyfriend, chances are high that a condom will not be used, regardless of high, low, or other.
Discos:
Most discos are full of *****s. The Thai middle class is very hard working (first generation of middle-class), studying etc. And nightlife has a much lower relevance than in most western countries. They would also not like to go to places full of tourists (especially sex-tourists) and *****s.
Nonsense. Try going to Thong Lor or Ekkamai on any night of the week, to the clubs where you will see 1% farangs, the rest all upper class Thais spending a lot of money and partying their brains out night in night out. Yes, you are correct, this class do not want to go to places with farang tourists, so they go to these clubs. But they party far harder and more regularly than their western counterparts.

why another summary on thailand / bangkok:
1) although bangkok is one of the places with most articles here, it is hard to find good summaries that give the reader an overview in a short time.
2) i used internationalsexguide as a resource on multiple visits to bangkok (and of course to plenty of other places!). unfortunately, i have to say that the impression and experience that i have from bangkok does not really match with lots of the articles here. i am a very frequent business traveller and was very disappointed from bangkok as a sex-destination. it is simply too low-level for me. this is definitely a subjective view, so don't feel embarrassed. i is not my intention to offend anybody here.
massage salons:
good things first! the massage salons are the best thing in thailand (of course, many other countries copied it). only a minority of salons offers "extra services", but at least in the red-light district (area sukhumvit 3 until sukhumvit 26, i don't really know patpong), most offer extras. a normal oil massage costs between 250 and 500 bath for one hour (i would book two hours if you want to enjoy smoothly). i would not look too much on prices (except you book in the hotel, which is almost always more expensive for no obvious reason). the difference between cheap and expensive are mostly the rooms and hardly the girls (the girls you see before anyway). the normal extras are handjob, blowjob and body-to-body massage. each extra costs roughly 500 baht extra (all 3 together usually only 1000 baht), but the prices vary and if you negotiate before (which many girls however find offending, they are quite shy over there) you can get cheaper and more "secure" (once your blood left your brain to strengthen your friend you will pay more and the girls know it) often. most salons that offer extras don't have such a professional massage as the non-sex massage salons, but the quality is usually at least ok. there are a few places that work like brothels and you can basically get everything, in my view those places are overpriced (starting from 2000 baht, usually 3000 – 6000 baht for bj + sex) compared to just taking a girl to the hotel. most girls will also join you to the hotel if you want, but prices are high compared to beer-bars.
beer bars & go-go bars:
beer bars are where all the sex-tourists hang out and – as the name says – usually drink plenty of beer. most bars have 3-6 girls that you can take home for a bar-fine of usually 500 baht. there are some bigger bars of course, especially beergarden in sukhumvit soi 7, where you only have freelancers and save the barfine. a beer is usually 70 – 100 baht.
go-go bars are almost similar, just beer is more expensive (150 baht), but many have happy hours where prices start from 50 baht. go-go bars are only in nana plaza (sukhumvit soi 4) and soi cowboy (between sukhumvit soi 21 and 23). bar fines are between 600 and 800 (you often pay more during happy hour). they offer dancing and so you see what you get, most dance topless, sometimes completely naked, sometimes dressed in underwear. however, the "dancing" is definitely among the lowest quality in the world. most girls absolutely don't know how to move and many write even massages in their mobile phone on-stage. i always thought it cannot be so difficult to do a normal striptease dancing. believe me, here you have maybe 40 bars and none of them is able to do this. when you've seen one of them you've basically see them all. you can easily go in into each one (no entrance) and check the action (= number of girls and maybe how little they wear, as i said, the rest is the same). they purely compete with the number of girls and the prices of beer, creative concepts are absent and you will also not really find more luxurious places etc. (nana has some bars on the top floor that sometimes offer fetish shows).
each bar (beer bar + go-go) has one (the bigger go-go bars often more) mamasan. basically this is the pimp taking care for the girls (and often not treating them nicely). it always works the same: the first 5 mintes in the bar, mamasan and one or two girls come to you, are nice, touch you a bit (it is usually not a pleasure to get touched by a mostly fat and old mamasan, maybe this is the reason why all tourists here drink so much, then they don't care any more). then they ask if you pay them a lady drink (costs usually approximately the same as a normal drink, few real rip-offs). if you say no, they make a couple of bad faces / tries to get a drink and leave you alone if you don't invite them. if you say yes, you lose the money of the drink and gain maybe 5 more minutes with the girl(s). they have to go on-stage "dancing" (they rotate every 15 minutes), so if you pay or if you don't pay won't make a big difference. consider just to the bar next door and do the same again.
the price of the girls (excluding bar-fine) is usually from 1000 – 2000 in beer-bars and up to 6000 in go-go bars (you can negotiate, but to get them lower than 2000 in go-go bars is hard), all for long-time / overnight. many girls, especially in go-go bars don't do long-time. prices for short-time are roughly half of this.
streetwalkers:
you find plenty of streetwalkers in sukhumvit, between soi 3 and soi 11. you can get long-time for 1000 – 1500 baht with nice girls, of course, there are always crazy ones demanding crazy prices, but hardly higher than 3000 baht.
hotels:
i can absolutely recommend the jw marriott and intercontinental in sukhumvit. both are very expensive for the area and not really special compared to marriotts or intercontinentals in other places in the world, but so far each of the definitely much cheaper local hotels had things that annoyed me (small stuff like low water pressure, noise level, cleanliness etc., but if you are a frequent traveller and used to a higher standard you will not be satisfied in the local hotels). a cheaper but reliable option is the ibis (there is one in sukhumvit soi 4 and plenty others in bangkok).
the local hotels are ok. i will not give the names of those hotels however because i don't want that anybody uses this data for the purposes of the following chapter.
minors:
unfortunately, this is – and here i disagree with many other writers here – a big problem. i realized in some of the local hotels (that's why i don't post names, i don't want that anybody abuses this information for the abuse of children) take the ids of guests, but do not check them. and i saw many abusing this and the hotels definitely know about what happens. in addition, several of the go-go bars (many of the ones recommended in this forum!) had girls with very questionable age. unfortunately, the police do not care at all: officially, prostitution is illegal and does not exist. it is one of the most corrupt countries and currently very instable and i am sure some of the "big guys" earn good money from being the world's biggest brothel. occasionally, the police arrest a foreigner and put it to the media, but if they would take it serious, they could arrest dozens per hour. i wonder that not more of the tourists here care and complain about it, my only explanation is that most of them are too drunk or not man enough to tell when things go the wrong direction. in addition – even though the majority of girls probably have full age – most of these girls come from very simple backgrounds and do not behave at all like having full age (one is the age on the id card, the other is how "ripe" a girl is). if you want to support such a place / country, it's your decision. but if you're man enough to have a "real woman", but don't want to grab the over-thirties, i think it is the wrong place (i recommend south america for this).

please note that all the prices mentioned for girls are girls that i would consider very good looking and around 25 years old. i suppose you get the older and uglier ones cheaper. i also feel that with age their service is sometimes better as they naturally lose a bit of their prudence and reservation in this job after a while.

cleanliness:
again a big point of disagreement with many of my fellow writers: hiv is a huge problem in thailand. it is true that the rates are decreasing, but they are still belonging to the highest amongst the world. as prostitution officially doesn't exist, there simply are no reliable statistics how high the rate is amongst prostitutes. but as thais are very prudent on sex, i would assume that the still high hiv rate in thailand mostly comes from prostitutes. certificates from the bars that they are tested? you can buy all types of certificates in thailand: in khao san road they sell you from id-cards to phd certificates on the street, the certificates even in most big jewellery stores are fake but if you want to believe in santa claus then i don't want to destroy your dream. but guys, take care! use your brain! if i read the "certificate stuff" here, my stomach turns upside down.
another nice experience i had was to go through nana plaza at around 5 pm around new year's eve (when i stayed the last time in bangkok). i've seen two girls there removing their lice. if you read reviews about local hotels in various websites, you can frequently find complaints about fleas in the beds, and they don't come from the hotel staff i suppose (another reason why you should take the international chains if quality matters for you).
what to do except sex:
- sightseeing / temples: bangkok has wonderful temples. just take a city-guide and go from one to another. you can waste much time like this and they are all worth a visit. what is really great is to take a boat on the river and drive through the canals. you can book a private boat for less than 1000 baht / hour (including driver and fuel) if you negotiate a bit. try to get rid of the middle man and directly discuss with the driver.
- shopping: on the streets and in many local shops, they mostly have the typical asian stuff: cheap, bad quality and mostly faked products. if you feel better with a faked rolex, this is the right place (and indeed, many tourists seem to love this stuff for whatever reason). the (street-) shops in khao san road are much cheaper than the ones in sukhumvit and especially in sukhumvit, you often need to negotiate prices (khao san is the place for backpackers who obviously know the local prices and customs much better). i felt that women usually don't overcharge tourists as frequently as men, so if you want to buy compare and try to talk to the female and not the male sellers. there are huge shopping centres around siam square and in siam paragon shopping centre there is a great food-court to eat. you won't find things cheaper than in europe or us, except maybe a few branded textiles that are produced in thailand. also electronic spare-parts (no-name only and not sure about the quality) you can get very cheap, such as replacement batteries for old laptops etc..
- tours: there are tours to the "tiger temple", elephant trecking and similar stuff. most are tourist traps and the animals are treated very badly, so don't do it! there are two historic cities (the old capitals of thailand) and a national park that are nice to visit and can easily be done on a day-tour. costs are around 1000 – 1500 per trip. you can do it also on your own for a fraction of the money if you manage the local bus-system.
taxis, tuk-tuks, transportation, and other tourist traps:
during the last 2-3 years, bangkok became more dirty, more disorganized and unfortunately you will feel it as a tourist. do not trust anybody that wants to talk to you (except the girls in the beer-bars that generally will say "welcome" when you pass by). just ignore, they cheat very professionally. if you need something, you need to ask for it, not vice versa.
same is valid for taxis. always take a taximeter, it is incredible cheap. you also can go from sukhumvit to the airport on toll-free roads for less than 200 baht. if the taxi drivers offer you a fixed price, be sure that it will be too high. the first two kilometres always cost 35 baht. it sometimes happens that they will not agree to take you for the normal rate for very short distances and on many tourist places you only find taxis that want to take you for a fixed price. just go 200 meters further!
the sky train and the metro work perfect, especially if it's raining i recommend them as the traffic can collapse very fast with rain. unfortunately, the network is quite small.
forget tuk-tuks. i can write pages about them. if you want to have a photo with them, drive once, ok, but then forget them! they are the main tourist trap in the city!
culture:
thai people are very religious, even most *****s treat each buddha-statue with great respect. it is respectless to walk with sleeveless clothes. thai people are also not very loud. unfortunately, most people in sukhumvit-area ignore all local customs. most thai will never say this to you (that's their culture and in addition you are their cash-cow), but this is disrespectful what most tourists do.
discos:

so be sure that the discos that you will go, many women are *****s (freelancers, etc.) and many of the girls from the establishments explained before go to discos later at night (most bars close at 2 am). therefore, whatever the girls tells you, be aware what you are most probably dealing with.
red light district:
it starts at sukhumvit soi 3 and goes until approximately sukhumvit soi 26. in most of those streets, you will find plenty of beer-bars and massage salons. soi cowboy and soi nana are the only places with go-go bars (plus patpong, that i however don't know much about as i always stayed around sukhumvit during the nights).

i just have to recommend this place! you pay 700 baht and get a blowjob. you can chose the woman (unfortunately, the quality decreased a lot during the last years, but you can find nice ones and it's anyway a quite dark place and you only get their mouth). the woman will then go up with you, you can undress her (most will keep on their slips) and get a wonderful blowjob. each of the blowjobs i got there was just perfect! they would probably earn more money if they educated our wives in how to do it than asking for 700 baht! they have excellent techniques and it's a good place to pass by and get rid of your pressure.
- condoms: you can buy condoms (and almost everything else you need) in 7-eleven stores. they offer condoms with "comfort fit" (or something similar not sure on the exact wording). take care as the "normal" condoms are for asian-size and not necessarily the best option, depending where you come from.
i hope i saved a couple of people from having wrong expectations about thailand, as i had when i came here for the first time some years ago.
for the people who know and love it (and there must be many), don't be offended. there are always different perspectives on the same thing.
