Always ask about the MSOG. It is standard, but at least the booker will know when he sends the girl, that you are looking for it in your service. (Good to have as proof should a dispute arises).

Tipping isn't needed, but that depends on the service you get. It's your decision.

Yes, the standard etiquette is for the customer to leave the donation in plain sight on the dresser. Most of them will take a cursory flip through it to be sure it is there and leave it otherwise untouched. Some will immediately put it in her purse. A few will do nothing but glance at it. If no donation is visible, most will ask for it, and that will create discomfort. This isn't a buyer's third-world market or a negotiation market where the guy doesn't pay until afterward because she might cash and dash. If he tries that, she is relatively likely to just leave. When you use a reputable agency, you know she is going to stay for the full hour. The agency has drivers who would not expect to pick her up until the hour is complete.

When the loonie and the dollar were at par, many agencies adopted a policy of accepting payment in either loonies or dollars. They have not changed that policy, but they do not do conversions! In other words, you can pay 200 loonies or 200 dollars. You would be a fool to pay 200 dollars because that is is much more money now than 200 loonies with the currency rate of 77 cents or so. It is definitely advantageous to convert and pay in Canadian currency.

Smuler is correct that the best lineups are often Monday through Thursday and the agencies report that is because a lot of the girls like to go out and have fun Friday and Saturday nights, and recover on Sunday. Here's a brief synopsis of my brief visit to Montreal earlier this month. Overall, a great trip and the city lives up to its reputation for being a good mongering place.
