It is called, Parque Lleras, not Park Illegal. It is not downtown, but rather in the more upscale sector of Medellin, Poblado. The chicas in Gustos will come down in price to 200-300 pesos if you wait them out, never negotiate in dollars. For me I found it really depended on the girl. Some would stay the whole time (even longer) but probably 2/3 did as you described. Not exactly running out the moment you finish, but after a little while asking if they can take a shower and then they get dressed.

Cash. Fase Dos, which is one of the better clubs will not accept cards. I can't even imagine whipping out a credit card in the El Centro clubs.

Your also dealing with 2 entities, the club and the girl. The girl is only going to accept cash, even if the club allowed you to run a tab.

And you're probably going to be unhappy the next day when you find out that tab came to $6,000.

The Real Beauty of being on the ground In Medellin for at least 30 days is that you can arrange many appointments with many of the beauties of Centro and Parque Botero. Someone can steal your card the same way they can steal your cash if that is what you are worried about.
