Speed (for some). Less clarity. More slang. Some words I have picked up and used (innocently) to my ex-wife and daughter (from Bogota) have resulted in very ugly looks and a "I would never say that" reply. Same for when I have discussions with a colleague who is an educated Paisa who's father is a retired OB / GYN in MDE.

The middle-class and upper-class are easy to understand in Medellin. But not the Castellano / Spanish spoken by the people from the poor barrios. It's like two different languages. I had a "monitor" at La Liga de tenis at the big Stadium in Estadio, a student. I could not understand what he said, he was from Santo Domingo, a very poor barrio,(also knew for it's futuristic library, Bibiotecta Española, inaugurated by King Juan Carlos himself). After tennis-classes I went to La UPB for Spanish-classes and one day I told my teacher that I did not understand my tennis-trainer. "Nor, do I, he replied".
Many prepagos are from those poor barrios while your Paisa-friends are more likely to be from the well-off barrios like Laureles, Poblado etc.
