I've Never booked online I always just walk straight to the visa kiosk it's right next to the passport control line. The line at the kiosk it's always empty or 2-3 people ahead of you.

Not sure about other nationalities but for us citizens it's $30 for 90 days stay.

And for taxi from IST to sisli or taksim which is like the downtown of Istanbul will cost you about 400-500 TL or about $30 and yes they take both but they do prefer like cash.
