Almost evey street in Istanbul, including most of the shantytonws, are cleaner than new york metro. If you ever used NYC metro, don't worry.

In terms of safety at the streets, it depends. Istanbul is a very dangerous place compared with, lets say Fairfax,Virgina, but it is much much safer than many city centers like Atlanta or Baltimore.
