All major places have a ATM. Accepting debit and credit cards. I was in the rather small Manakara the had several to. I did not bother about the commission since it was my cheapest fun vacation anyway. I would propose, stay away from large amount of cash for security reasons. Store the cards in different places. Most of the girls try to serve you in all manners without asking for to make you happy.

I also got the impression that having some €uros would be a good idea. I'm thinking about bringing 500€ in 10€ notes for easy transactions and having a cash "reserve" if the need arises.
When it comes to communication it seems like using direct translation on phone should be sufficient to make deals with the working girls, which is good enough for me.
I'd appreciate any advice on picking up (working) girls apart from the hardcore crew from Taxi Be. Like many others I prefer slim girls in their 20's.
Going rate for short time seems to be 20-30k and overnight / long time rate 40-50k?
Lastly if anyone would let me know what "rimjob" is called in Malagasy and French I would be eternally grateful.

Since I don't speak French I'm having a difficult time confirming excactly what the restrictions on Madagascar are. I think I am allowed to travel to Nosy Be from Sweden (I can book flight with Air Ethiopia changing flights in Addis Abbeba). It seems to require a negative PCR test no older then 72 hours.
I also have a question about hotels. I have made a preliminary booking at hotel Kokoloko, which looked like a step up from Ambatoloaka, but I would like a confirmation that it is a guest friendly hotel.
Also, I have finally registered a Bitcoinwallet and I just made the membership deposit in support of this great community.
