You just need to ask the woman the entrance on level 2 who puts the tag on your wrist. She keeps all the forms to fill out in a drawer at the desk. The forms are in several languages including Hindi.

But usually there is no exclusion from STDs. It is a medical problem like other ones. Moreover usually is still cheaper to pay that a big incident.

In this case of little payiment some insurance pay directly the hospital, and other will refound you.

Only exception: if you get the STD before you travel. So DON'T tell this stupid shit to the doctor (I read in other comment).
