There are several DH places in Okinawa that I am aware of which accepts foreigners. There may be caveats though. Such as not all the girls in the shop are willing to service foreigners. Timely and well-written post. Definitely backs up the adage "A little language goes a long way". Buckled down on the Nihongo skills 18 months ago with this primarily in mind. A good option would be to make an online reservation (for example, create a free account and use a customs site which compiles services such as, CityHeaven - https://www.cityheaven.net/). Making an online reservation allows you to provide many details without the need to have long (er) phone conversations. Also, if you use a web browser such as Google Chrome, you can jump back and forth between English and Japanese to verify your input before submitting. Whenever I have made an online reservation, I have generally been given a confirmation call within 5 or 10 minutes. So be prepared to take a confirmation call after you hit submit. I also try to play in the area where the shop is located. If the shop is in Naha city, I'll choose a hotel somewhere in Naha. If the shop is in Okinawa city, I'll pick a shop somewhere in OC, etc. I've had ladies from these stores and I suspect there are others; I just haven't tried yet.

I've tried three different Massage Republic providers in Okinawa. At various times these providers also operate in other parts of Japan. Generic massagerepublic website https://massagerepublic.com/.

Following is my experience:

Sawa communicated well via Gmail and her website information is mostly accurate. She does work in a medical field but her website statement of "finished med school" doesn't mean she was a student studying to be a medical doctor. More like a medical technologist. Sawa suggested a love hotel in Chatan as an in-call site and told me the price of the location. I assumed that I would have to pay for the hotel as well. However, she quoted me an incorrect price (it was 500 yen more than she said it was going to be) so as an apology to me, she paid the full price of the hotel. If anybody looking for wild, young, and free then there is a club in Naha called Clutch (club-clutch.com). This place, before COVID, is literally off the chains. Chicks come to get their "groove on", even right there at the club.
