Minato ward is definitely in Zone A, so you won't need to pay extra for transportation.

The normal way in Japan is phone, but I've used Asian Mystique once and only contacted them by email. The girl will need your phone number though, for the meeting point.

Hotels wouldn't usually be an issue, but it's wise to meet the girl somewhere outside the hotel and then bring her to your room, I think.
