Looking to be back in KL later in the year, for the first time in about 3 years. Was wondering if anything has changed significantly in that time.

I would say it's unlikely, but not impossible.

Stardard rate requested is 500 MYR, paid by the majority of punters without an eye blink (what a disgrace). I often managed to negotiate down to 400, but lower than that it must be an old one, who nobody wants, or someone with who a special feeling has been established (it may happen, and it happened to me).
Usually, I personally leave out those who stick on 500 (the probability of getting a doll who performs a mechanical procedure is high), but I tend to accept 400, as I feel that she is sacrificing some of her usual rate, and therefore I like to think that she likes me, or she is motivated by the difficulty of finding another customerr (offer over demand).

Try your luck, and, as always, YMMV.

My MO is offer 300 and 50/100 tip for good perf. Last year all took it. Maybe if they reject this offer 350/400 + 50/100 and see what happens.
