There are karaoke bars, massage shops, and brothels in just about every Thai city and town. The tourist business is limited to a handful of well lit, well known locales.

I've seen plenty of local places in small towns. Business was terrible.

Pattaya likely makes more in a week than the the domestic places in every small town in Thailand make in a year.

The Thai government itself has admitted Thai IQ's in general are below international standards, and Isaan is well below even the Thailand average. The Thai government then takes the politically correct stance that low IQ's are fixable, when in fact beyond marginal IQ improvements with better education and nutrition and so forth, IQ's are very slow to change, as their causations derive over thousands or perhaps hundreds of thousands of years. Now as far as Bangkok and other areas such as Phuket, they have benefitted from a 1000 years of Chinese migration and intermarriage, which of course acts as an offsetting factor given that China, Korea, and Japan on an aggregate basis have the highest IQ's of any group in the world. As has been widely reported "more than 80% of top 40 richest people in Thailand are Thai of full or partly Chinese descent". Raw IQ being largely innate from birth may be unfair, but it can and often does influence what type of occupation one ends up working in.

Isaan on the whole is not as backward as people often think. It is the home to more than 25 million people and there are large cities (Khorat, Khon Kaen, Ubon Ratchathani, Udon Thani, Nakhon Phanom) , with thriving economies and good schools. The countryside is mainly agricultural and seasonally dry, however, it is very productive in the wet season. The generic poverty that I've observed is more related to lack of infrastructure investment (roads, etc.) , poor quality education, and restricted access to private capital. Of course, peoples' habits and bad luck can create personal poverty.

In general, Thai men prefer young girls who've not born children, and with light skin. You don't find a lot of Isaan women who meet these criteria. Most of the white skinned women come from Bangkok, the north and central regions. Also, Isaan women generally speak a different language (Isaan / Lao) than moneyed Thai men.

Even in Chiang Mai and Hat Yai? Perhaps not.

Thai people will go wherever they can to make money. I lived in the south for awhile and met a lot of people from Isaan, and other parts of Thailand, as well.

Chiang Mai is close to China in more ways than just geography. White skin, beauty, and elegance are valued here in female characteristics.

Malaysian men, who form the predominant group visiting frequent Hat Yai, probably are not as interested in white skin as their Thai counterparts.
