It's definitely a translation issue. But I think when written in Thai, it's supposed to be a kinder term than Ladyboy or Katoey. So it's second woman, not original woman. But it gets more complicated when the poster explains that she converted to a woman (meaning removal of penis) or not converted (retained a penis).
