Those are all good questions. I have taken to directly asking lazy hookers why they are in the business if they never actually DO anything and say NO to almost anything a customer would actually want sexually.

Of course, it's hard to argue and shame a girl out of her chosen profession if she can make 2,000 - 4,000 baht, more than a week's pay, for doing almost nothing pro-actively in a hotel room for 30 minutes.

How is it that the guy who films these videos is always as if he is invisible? The girls don't even glance at him. It's the same when he's waking the streets.

These cameras can be as small as a bottle cap. The lens at least. In this case, it appears from the low, waist high angle that he put the camera in a fanny pack with a hole in it for the lens or perhaps even on or near his belt buckle. Nobody knows they're being caught on video.
