I'm up to the payment screen and it doesn't mention the name of the service (Nexus Tours or otherwise) or where the pick-up location at the airport is. Question: where / how does pick-up work at the airport? And on the flip side, where / how does pick-up on the return trip work?

Reading all the reports in here, I'm really considering going to BB soon. Have been to the DR before but never to BB. I know many must have asked this before, is BB the best in DR? Looking at the rate they are offering, I can't find anything similar that can compete to BB.

I've been missing BB's lately and Andy would be almost at the top of my list if I ever get to return. I'm crap with names but she'd often pal around with another girl who would be top of my list. My trip started like a lot of people's; got there late at night and got immediately pounced on by a couple of the girls! I had a few hundred dollars on me so took one of the girls for short time. She gave a good BBBJ and I slept like a baby.
The next morning I went to Jenny's to exchange some Euro. They brought me into the back and asked me if I was okay with the rate before completing the transaction. I got a little less than the current exchange rate, but more than most exchange places were offering. Jenny's also has an ATM. When I was there the limit per card was 10,000 DOP, and it was frequently out of order. I suspect it just couldn't keep up with the demand and kept running out of cash. It's also convenient to get some things in the market. Toiletries (I had forgotten to pack mouthwash) or drinks and snacks (cheaper than the mini bar). It's also nice to get an ice cream while you're there! It's about a 15 minute slow walk to the market. You could get a moto to bring you, but I preferred the exercise and security is not an issue in Costambar. The only issue I did have was one time a couple of police officers stopped me to look for a "tip". I pretended to be a dumb foreigner who didn't speak Spanish (mostly true) or English. If the ATM is out of order you can get a taxi to an outside ATM for a small fee. If that happens there'll likely be other guests in the same predicament, so you might be able to share a cab.
I was there for two weeks, so boredom was a bit of a factor. You can't spend all day fucking! I did a lot of day trips with a local tour company called Iguana Mama, some of which they sub contracted to others. The highlight was the 21 waterfalls tour where you jump your way off waterfalls down a river. Great fun and almost worth the trip by itself! I did snorkelling, ATV riding, the city tour, etc. Some tours I got picked up or dropped off by the market or gate to Costambar for discretion, but some insisted on picking guests up at the hotel. Another aside here is if you do the city tour and visit the 'cigar factory', buy some juego mama there. There's vast differences between brands and my biggest regret is not buying more than two small bottles there.
It's good to get out of the resort during the day. There's less girls around during the day, and if you are worn out from the night before (like me! It can be annoying when they won't take no for an answer. They would never bother me too much, and go away when I said I'm too tired, but another girl would try within minutes, and the first girl would probably be back within an hour. You can't blame them really. They're there to make a living. I can't say that the ways they tried to convince me were all bad either. Mostly if I wasn't out on a day trip, I'd hang around the beach, get a taxi to the FixCi gym or hide in my room. The TV in my room played videos from by USB stick, so I was able to watch TV shows and movies I had saved without resorting to my tablet. The cable was also relatively good. I was able to watch the League Cup final in my room. Taxis charging the gringo price could be expensive on my own, but somebody will quickly hook you up with a local taxi driver who will do it cheaper. Hanging around the beach can be a good way to spend a lazy day or two as well. I met two guests at a beach bar / café one day and we ending up chatting for most of the day. The (many) beers and lunch we consumed during the day cost us $15 a head including tip. Of course if you're not worn out there's just as much fun to be had during the day. The same girls rotate and do the day shift so there's plenty of attractive women around during the day too.
The food at the resort is okay. I think the problem is supply chain and lack of quality ingredients rather than the chefs themselves. I always found even eating at expensive restaurants (eye watering by local standards) was underwhelming. Breakfast and especially the fruit plate was usually pretty good, and for price and convenience I wouldn't really knock the place for lunch / dinner. There's an Italian place around the corner that is better for an evening meal though (SoleMio), and some of the guests liked to get breakfast in Rick's Café.
I've gotten this far and hardly mentioned the girls! The answer is varied. Sometimes you get out of the experience what you put in, sometimes it depends on the girl themselves. Most of the time if the girl has been doing well lately she'll probably put minimum effort into everything, compared to a girl who hasn't had many customers lately. Sometimes if you have a girl you pick 3/4 times you get less and less effort every time, sometimes she learns what you like and it gets better every time. The best experience I had while I was there was after one of the Friday night parties. A lot of the girls love the parties and love to dance. If you're a good dancer you'll be popular with the girls. If like me you're a terrible dancer but make some effort to move a bit the girls will still appreciate the effort, and it's never bad to have lots of girls in tight outfits grinding on you! I spent a lot of time that night with another girl who would be high on my list (Yana) who I'd been with a couple of times before and that top-of-the-list girl whose name I can't remember and brought both back to my room for the night.
When you're there for two weeks you get to realise there's some infighting amongst the girls. Don't even try to get involved! They're competing amongst each other and some women just aren't going to get along with each other anyway, so just stay out of it. Not your problem. Another possible thing to avoid is when you're looking for two girls allowing the first girl to pick the second. This can be tricky. You want the girls to get along and don't want to pick two girls who hate each other, but sometimes the first girl will pick a friend who hasn't had many customers lately, who might not be top tier in terms of attractiveness. It might be better to suggest the second girl to the first and see her reaction.
I have to go soon so I'll try and finish off on a positive note. Enjoy the events! Go to all the parties in the Crow's Nest and don't miss the bikini contest if you're there at the time. I never got to see the wet t-shirt competitions and regret missing those, but I did get to be a judge for the bikini contest and it's something not to be missed! I've heard that lately the judges kick in some money to make the contest more enjoyable, so it must be even better now.
