By overturning the abortion-rights decision Roe v. Wade, the Supreme Courts conservative majority has added fuel to calls among some Democrats to change the court itself. Some want President Joe Biden to pursue the idea of expanding, or packing, the court beyond its current nine seats. Others would replace the life tenure of Supreme Court justices with fixed terms to guarantee each president has equal opportunity to shape its makeup. In December, a commission appointed by Biden reported back on these ideas, among others. By all indications, Biden seems unenthusiastic about pursuing such radical steps.

In 2020, while a candidate for president, he ruled out term limits for justices, saying, Its a lifetime appointment. Im not going to attempt to change that at all. He also said hes not a fan of court-packing, which years ago he dismissed as a bonehead idea. To Americans angry about the Supreme Courts composition and rulings, Bidens response has been that they should elect more Democrats to Congress.

No, they didn't. But who did lie, on a more important and indeed existential matter for the country, were Senator Edward Kennedy and his cohorts who sponsored the 1965 Immigration Act. A study published this week in The Lancet showed a high mortality rate described to the media as "horrific" by the study authors among older patients, particularly those with underlying conditions. By comparison, no patients under the age of 30 died at the two hospitals, and only a small number in that age group had to be put on ventilators.

In April, another study published in the medical journal JAMA looked at the outcomes of 5,700 patients hospitalized for COVID-19 in the New York area, finding that only 3. 3 percent of 1,151 patients who required ventilation had been discharged alive as of April 4, with almost a quarter dying and 72 percent remaining in the hospital. This is a significantly higher death rate than what's usually seen for patients put on ventilators for other reasons in the past, such as bacterial pneumonia or collapsed lungs.

The main reason people with COVID-19 have required ventilators is that they developed a condition known as acute respiratory distress syndrome (ARDS), inflammation in the lungs that makes it extremely difficult to breathe. But there's a specific subgroup of patients known as "happy hypoxics" who may have different needs.

These are people who show up in the emergency room with terrible oxygen levels, whose X-rays reveal severe pneumonia, but are still able to talk and whose only real complaint is some shortness of breath," she explains. For these patients, although their vital signs indicate they should be on a ventilator.

Instead, doctors now try giving these patients less invasive treatments, such as very high doses of supplemental oxygen, either through a continuous positive airway pressure (CPAP) machine, the same machine used to treat sleep apnea, or through a supercharged oxygen system known as a high-flow nasal cannula (HFNC).

A paper published on April 22 in the journal Academic Emergency Medicine found that when hypoxic patients were given these types of supplemental oxygen and positioned flat on their bellies, only about a quarter ended up requiring ventilation. Kaplan says this approach not only appears to improve patient outcomes, but has also prevented the overuse of ventilators, averting the ventilator shortage physicians had originally feared.

What the article has wrong is that there were not just a "subgroup" of happy hypoxics but all patients with Covid, and putting 25% of patients of hypoxic Covid patients on a ventilator did not happen in the future as doctors got more comfortable watching these patients. Subjectively, it was when patients said they doctors considered ventilators. And objectively, elevated carbon dioxide in the blood was a marker that someone was with impending respiratory failure and was likely needed to be put on a ventilator.

Putting a hypoxic Covid patient on a ventilator solely for "pumping oxygen into the body" was committing malpractice. Period.

OTOH, if you and Viliany have low oxygen levels and Covid, by all means tell the doctors how smart you are and request to be put on a ventilator. Just tell me before hand so I can buy a life insurance policy and cash in on your demise.
