Tests are always the first step, the docter takes a swab or you do a "first catch", test. That is, what (the politically correct word police won't let me put down the word for going to the toilet), you first let go after you wake up in the morning. Get that through the labs and you are at a starting point as you can see what you are dealing with.
You would be surprised how often it is something else. But! Your symptoms sound disconcerting. I would recommend the tests. You could probably fly to Singapore and get checked there for less than the cost and risk of a dodgy test in Phils. Countries like Singapore take public health very seriously and would certainly have a comprehensive response if you indeed have something. Sometimes the discharge is just dead DNA, so the test will detect chlamydia but you have already killed it if you have been eating azithro.
The other aspect of STD's is that of your mental health, you can feel like a total loser and having to lay off sex can drive you double crazy. Relax!, those bacteria were around way before you got here. Julias Ceaser died of syphilis. Its not so bad, you will get better if not already thanks to modern medicine and you are still a good guy. I did indeed go to Chong Hua for one round of tests. (after Hi-Precision came up with nothing) And they recommended me to the infectious disease doc I'm seeing as she's apparently one of the five best in Cebu.
I'm sure whatever I've got is all over the place. There was some French guy sitting next to me in the waiting room 2 months ago when this started, the whole time flipping through his Tinder matches. He went in before me and the doc said he had the exact same thing but he didn't appear to be slowing down.

I would first follow WR's directions.
If 3 doctors gave you the same advice, medication you could try another. You can try an Urologist specialist or another Internal Medicine or Tropical specialist and get a full STD panel. A post or pre treatment test is a better way to see what is really ailing you. Take pictures and you can take you own sample to show them that it is really there. Just use a clean glass jar. I would also wash your underwear in hot water and use bleach.
I prefer to see US doctors or US trained. There is a medical evacuation clinic in Manila that might have some on staff. Also, it sounds like you travel. In your home country there are probably specialist clinics dealing with STDs. Good luck. Use a wrapper, my 2 cents.

So I've been in Cebu most of the past year. And had a lot of unprotected sex with girls here. That was 2 months ago. The symptoms abated somewhat. The pain went away. But the itchiness, inflammation and discharge never completely disappeared. I saw a different doc. He gave me another shot of ceftriaxone in the arm and a prescription for doxycycline. I can't remember now but I think I might have only gotten a week of doxy when you're supposed to take it for 15 days, maybe that was my mistake. But anyway symptoms went away for about 5 days then came back. Doc gave me another gram of azithromycin.
I went to see an infectious disease specialist. She ordered a lot of expensive tests. I still haven't gone for a gene based urinalysis which she said would be the best way to detect chlamydia because it costs 8000 pesos here. But I had two swabs and gram stain tests with culture and sensitivity studies. The first was all negative the 2nd found some bacteria but the doc said that it was the most common bacteria normally found on human skin and so probably they just scraped it off the inside of my dick. I had an HIV and syphilis screen, negative. Hep screen negative. Tested for diabetes, my thyroid, all normal.
When I was at the doc she wanted to see the discharge but it's so minor, usually only noticeable in the mornings or if I haven't taken a [CodeWord140] in a long while, that she couldn't see anything. And she acted like I was just imagining things. But I know something is still wrong. I still have the ithiness, the inflammation, and the discharge.
She told me if I went and got the expensive urinalysis and they found chlamydia they would just prescribe me more azithromycin. But what would be the point of that when I've taken it 3 times already. She said it could be reinfection. But I haven't had sex in 2 months. She doesn't think it's antibiotic resistant chlamydia because she hasn't seen cases in the Philippines before, which just seems ignorant to me since of course there are lots of sex tourists here bringing all sorts of things.
Has anyone here experienced this? Did you find any treatment that worked? I read online a story of some woman with chlamydia that was resistant to azithromycin and she treated it with a combination of 3 different antibiotics but she doesn't give the doses.
