Leonardo Boutique is definitely not within the more interesting Tel-Aviv neighborhoods, so lower your expectations there.
Mike's Place has several branches. The location next to your hotel is catered mostly by the nerds roaming that area. The one the that you are looking for (and has been mentioned in Skogis report some time ago / see below) is in the touristic area on 86 Herbert Samuel, near the US Embassy.
From what I remember about your hotel area, the only transportation choices there are taxis or buses; a quick glance at the buses' map shows that there are several main lines fairly near to your hotel: 12, 22, 52, 89, 141, 189, etc. , though you better ask at the hotel. Waiting for the bus can be quite time consuming, and taxis are expensive. Ultimately you'll have to decide between the two. For example, a taxi drive from your hotel to Mike's on Herbert Samuel lasts about 15-20 minutes and can cost you say 50 Shekel, while by bus it'll be about 1 hour and cost 7 Shekel. Each way.

I have had Escorts come to the Boutique before with out issues. Cab is your best bet to Mikes at the Embassy (bring a business card from the Boutique) some drivers didn't know where it was. You can count the better ones on two hands, and the ones that fit to my taste to one hand. I've also noticed that a few agencies start by doing things right, and it takes some time until their service inevitably declines. I had some 5 years ago an excellent experience with one of Rose girls, but after 2 or 3 disappointments, I came to same conclusion as Tylerpd.
