I always bring chew-able Immodium (anti-diarrhea) , pepto bismol in the tablet chew-able form and chew-able gas-x (I always carry-on some of those tablets on the airplane, not fun having diarrhea on an airplane or the farts, LOL).
After drinking loads of presidente beer my stomach gets gassed up. Depending upon the chica I am with, I generally don't like to let out a long, loud fart while she is next to me.

I don't eat there now after I noticed that when they come out of the kitchen with new supplies, they don't remove or clean the serving bowls / platters. They just add the new stuff on top of what's left of the the old. You can imagine that by the end of the day there are stratas of food at the bottom which have been hanging around all day. Not good.
More generally, even when young and fit I never had a particularly robust stomach. First two trips to the DR five years ago I had stomach trouble. Since then on two trips a year, and living here now, only one episode (as above) so I think you can get acclimatized and build up resistance to the local bugs.
Of course, I cannot be sure it was the La Sirena food. They have a pretty good reputation and there is a wide selection at cheap prices. But I am not taking any more chances after my observations above.
You are no alone in your views. I've herd too many sad sad tales about this. The only ones I hear that workout well are when the dude saves properly for his retirement and moves to DR lives like a KING for the rest of his days. Or when a dude gets a chick from China. Since this is not the China form that doesn't apply to us here.
