I don't have the extensive and worldwide experience of the experts here, but I'll toss out what I have.
When I went to Bogota I only took $400 in cash, with plans to just use the ATM for my needs. My hotel was paid with my flight as a package deal, so I didn't expect to need much.
At the airport ATM I had my first problem. Even though my card has all the affiliations to make it accepted anywhere, it wouldn't work. The next day I tried every ATM I saw, still no luck. I contacted my bank, but they weren't the problem. Finally I found a Citibank office and it worked fine.
For my DR trip I took all cash. About half was hidden in different spots in Mt carry-on and the rest was on me. The majority of my cash was always locked in the room safe, with a small emergency fund hidden in an unlikely place. I only changed over $100-$200 at a time and only carried what I needed plus a little extra, when I left the room.
I always split my cash between 2 or 3 pockets, depending on how much I'm carrying. If someone picks your pocket, they won't get it all.
