I also want to make it clear, I am not referring to products that do not contain actual synthetic testosterone but claim to increase testosterone levels and sex drive. My remarks are limited to synthetic testosterone available only by prescription.
First, three groups of men take synthetic testosterone: (1) athletes seeking improvements in their performance; (2) men with abnormally low testosterone levels for their age; and (3) older men who have normal testosterone levels for their age but lower levels than when they were younger.
Men who report having a low sex drive are more likely to have low testosterone levels. For example, MIT conducted a study: the average age of the men was 47; 11% of all the men in the study reported a low sex drive; among men with low testosterone levels 28% reported a low sex drive. Of course this only establishes a correlation between low testosterone and low sex drive, but you will be hard pressed to find a credible scientist who does not believe low testosterone levels are related to low sex drive.
The use of synthetic testosterone to increase a man's sex drive is more complicated. Broadly stated, many men with low testosterone who take synthetic testosterone report an increase in their sex drive. This is an empirical fact. The specific physiological mechanism responsible for this is not understood at this time. There is a plethora of research that shows synthetic testosterone increases physical strength, muscle mass, bone density, elevates mood, promotes more regular sleep and reduces body fat. The mechanisms that produce these effects are well understood. Stress, poor sleep habits, depression and other psychological factors diminish sex drive. Synthetic testosterone's impact on these factors could explain why some men report an increase in sex drive from taking synthetic testosterone. Of course there could be a more direct connection between sex drive and the elevation in testosterone produced by taking synthetic testosterone.
This is strictly anecdotal but I know several men in their 60's (about my age or a little older) who take synthetic testosterone. As a result, there has been a noticeable change in their physical appearance: improved muscle tone, reduced body fat and improvements in the appearance of their skin. They claim they have made no other changes in their life-styles. They also claim to have more energy, feel like a "younger man" and to have a greatly increased sex drive.
There are potential side effects from taking synthetic testosterone. It is now known that some of the reported side effects (such as testicular atrophy and increases in the size of the male breasts) are not attributable to testosterone but to the concurrent use of anabolic steroids by the men in these early studies. The biggest risk is that men who have prostate cancer will worsen the disease; therefore, a family history of prostate cancer is a contraindicater.
Artificially manipulating testosterone levels is a serious matter. Anyone considering taking synthetic testosterone should bear in mind that the long term effects are not know at this time. Do your research, talk to a couple of doctors and think carefully before making a decision.
In fact a recent limited study suggests that many symptoms commonly treated with testosterone are actually a result of low estrogen in men as both men and womens bodies contain both and the balance is essential. And, that testosterone dosing often has no meaningful results yet potentially harmful effects.
One should be cautious regarding medical conclusions offered on this and all anonymous sites.

Freud thought it pertained to everything that we would subsume under the word love. Sex, as we think of it, is just one manifestation of libido. Like many terms that Freud coined it became widely used by other scientists (Freud was a medical doctor specializing in neurology) and laypeople. In the medical community sex drive and libido are interchangeable. Low sex drive (libido) is a subjective term. A doctor would say a person has a low sex drive if he perceives his libido to be low and is distressed by this. There is no external, objective way to validate this person's self-perception. Oddly, this implies a fellow in Bangkok could be said to have a low libido because he believe his sex drive is low because he is only motivated to f*ck one woman every day and is distressed because other guys he knows f*ck two or more woman every day.
If you are horny every day you would have no way to experientially relate to the term. Lucky you.
For what it's worth, the guys I know who take synthetic testosterone are adamant increasing their sex drive was not the reason they started the therapy. It was for the objective benefits: increased strength, muscle mass, bone density, etc. In fact a couple guys I know complain about the increased sex drive. They're married to women their age who are generally disinterested in sex. This was not a problem for them before. Now they tell me, as when they were younger, they get horny looking at young ladies and are constantly fighting temptations.

If you have Low T, or think you have it, go to a doctor and have them run the necessary blood tests. These look at all of your hormonal markers, not just Testosterone. High levels of Estradiol can also make a difference to a mans mood, and to his body, for instance. Only a blood test and a good doctor will give you realistic options to consider. And don't compare advice from young bodybuilders using steroids as being informative to older men using Testosterone (along with anti-aromatising products) in order to maintain a good quality of life. They are as different as night from day.
Let's be healthy, be educated about this, and support people in making good choices. Testosterone replacement needs caution, to be sure, but it offers help in the same way estrogen replacement helps older women.

They are taking it under physicians' supervision.

While perhaps muscle mass and other issues can be improved, the big issue and really the central focus and implication of their marketing campaigns. Low libido. Has yet to be resolved. Step back and apply commons sense: if the drug companies had the science (beyond anecdotal reports) supporting that "low T" intervention actually worked in regard to the libido issue, they'd certainly have the monetary motivation to be out front pointing to these studies in the medical literature. The reality is these studies do not exist.
