I found the sign in English today. The 200 rmb price is an Expo special and the normal price is 380. I have been several times with 2 attendands and there was never an upsell from straight massage. Sorry for the misinformation.

The "real deal" high class escort is not something that you easily find here. If it does exist...and I'll bet it does...then it's probably very private. I'm not aware of any networked corporate services like this in Shanghai.
The closest I've seen to high-end take out is at the bat in the Shanghai World Financial Center. Under the right conditions (they trust you, know you, and you ask for some overnight company) the staff just might have the phone numbers of some high-end treats. Probably will be Russian or Easter European. Not Chinese. This is not surprising given the wealthy clientèle in the building.
But as for high-end Chinese, I'm not sure they have that.
A theory...and it's only a theory. The best of the best Chinese girls can become the second wives of the wealthy Chinese. They don't have to learn English if they are second wives. They have mega cash flowing to them and don't have to worry about much else, except looking like yummy arm candy when she's with her money-man and he's showing her off to his friends. Kind of like a nice car, watch, or other accessory.

A couple of points about the KTV girls you mention:
1) You will never see them in a line-up.
2) They are private stock for the really high rollers. You need to go there with one of these guys.
3) Many are booked by phone in advance.
4) Minimum is 1k RMB sitting fee.
5) They never go out the first time you meet them
6) They charge 10k RMB and up for FS and they expect expensive gifts on top.
7) As a laowai, you have little to no chance of doing them.
8) The best ones end up as second wives pretty quickly.
