All in all, in vaginal sex the risk is like 1/1000 assuming she has it. It becomes 1/10 000 if there is a 10% chance she has it. That is almost impossible odds. And 10% might be too high in Thailand.
The most recent UNAIDS report (2012) listed the Thai female sex worker HIV rate at about 2 percent. Compared to around one percent of the general population.
An HIV infected person on the best & latest meds is expected with high probability to live a full life span, just like any other person. So a 20 year old to around 70 or 80 years.
I think your policy of selecting girls who are less likely to have STI is an excellent one. I like to BB those who are extremely reluctant to not use a condom. It sometimes takes a few sessions before they finally give it up. Other times they do so on the first date when I show my recent HIV test results & or promise not to cum in them. These things are for her safety.
For your own safety it is quite quick & inexpensive to go with your lady & get her HIV tested at Bumrungrad hospital before having BB sex. They have results in one to three hours. If she tests negative this affords you safety vs HIV comparable to using a condom, without the need to use one.
Additionally, for my own protection, I am circumcised, which also provides significant protection vs HIV & other STI.
If you are interested in more details about any of this, there is lots of discussion in the Thailand Safe Sex thread.
