If you are from the USA or most European counties, you will gain entry for up to 30 days without a visa. There is a website that explains entry requirements.
At medical documentation to get into BKK? . I have never heard of this drequirement for medical documents -- unless you are coming from a West African country where Ebola is a problem, I don't know what this requirement might be. For a residence visa maybe?

I think the fee is about 1,000 THB or US $30. I don't think I every used this service -- likely you can pay on the spot at the airport.

Most of them are quite friendly and great long terms companions once you have managed to scratch through the ex soviet surface hardened tough hide.
25 years of visiting Thailand, I advise caution for the uninitiated
Why? Because it can become so damn addictive. Also, it has caused me to lose interest in western ladies.
Here is how I do Thailand, you may take my advice or leave it. Free advice is worth what you pay, but I have also found that truly good advice comes from unexpected places at unexpected times:
1) I don't do Soi Cowboy, Nana or Patpong, it's just not my thing. It's a bit too mercenary (and loud!) for me. Also, I can have a great time at many other places like the small bars along Soi 26 or some of the other smaller night time venues.
2) Each day and night should have a different theme. One night might be Ratchadipisek high end soapie night, another night might be a dinner cruise up the CP, the next night might be buying a few ladies out of a massage parlor, etc.
3) Stay hydrated, sleep well, leave all valuables in your in room safe, don't get drunk and stupid (or try not to);.
4) Do make lots of friends with lots of Thai ladies. You do this by going on dating websites, getting to know them, calling them, talking on the phone, and then taking them out to dinner when you are there. Do not treat them badly online, Thai ladies don't like sex talk. Be friendly and polite, meet them in BKK, and woo them to bed on a date. Never send money to them, ever. If you want to give them a gift when you are there, that's fine. But never be a sponsor, ever.
5) Try not to fall in love. If you suspect your Thai girlfriend has other men on the side, she does. In fact, it's probably much worse than you think. But that's OK, and that's fair, because you aren't exclusively seeing her.
6) Don't go to Thailand unless you have brought sufficient funds. There is nothing worse than being in The Land of Milk and Honey, and not having enough money. You should have enough money to do what you want, when you want.
7) Wear condoms for intercourse. BBBJ is fine.
8) Don't get into disputes with Thais. If the taxi driver is cheating you 100 bhat or whatever or your check bin is padded, let it go. It's not worth it. If there is a major dispute, Thais stick together, and it won't go well for you. And when and if the BiB show up, they will probably arrest you.
9) I like massage parlors. Get to know reputable mamasans and papasans. Get their email addresses / phone numbers. If they are good and know their stuff, they can really get you what you want. I got to know the papasan at Poseidons (Lucho), during the Viet Nam War, he worked at a US airbase in Korat. He wears a ring given to him from a pilot who was shot down. We used to trade emails on occasion, and he would tip me off to good performers and new girls. He always knew what he was talking about.
10) Get to know massage girls as well. For example, I am fairly good friends with Saki at All Stars (she works out of several MPs) and I have known her for years. Sometimes she will set me up with her friends or have threesomes or even foursomes, and it is cheaper than doing it through the MP.
