As prostitution is heavily frowned upon, the police tend to check girls with foreigners. They tell you over and over, having a relationship with a Cuban is not illegal. Buying sex is. Once a girl is caught / registered with a foreigner, there is a record and if she is with other foreigners. Well, it is an issue for the girls.

That said, cops often can tell or know the girls that have been caught before. Then there are the dead giveaways i.e. 100 year old guys with 20 y.o. girls

They also tend to shake down girls.

I don't know if it still is but at one point parts of the malecon were protected in that a group of girls were paying the police off, who in turn were keeping the area "clean" for them such that only they could ply their trade in that stretch.
