what do you exactly mean by "rural Thailand" ?
If you are talking about cities in Isaan or anywhere else you shouldn't have a problem. But the scene mainly consist of MPs, karaokes and coffee shops with singers and hostesses, no open bar scene except in the main tourist areas like Chiang Mai, HuaHin and so on.
Even in smaller towns like Prachinburi, Kabinburi, Sa Kaeo, Prachuap Kirikhan and so on you will find company. It might be less obvious if you don't know what to look for. Many of those places have these colored "christmas tree" lights. This is always a good guideline for a potential P4P place.
Talking Thai is of course a great advantage at these venues.
But if you mean a small village in the middle of the rice fields like "Ban XYZ" - which is rural Thailand for me - than you won't find anything.
In short, any place that is at least an "Amphoe" might have a venue with providers. Smaller places that only have the status of a "Tambon" or just "Moo Ban" most likely won't fit your needs.
From my experience you can find a lot of p4p everywhere outside little villages etc. in form of karaokes you will recognize by the coulorful flourescent lighting. But as professional as these girls are they do not usually have any falang clientele and it will be up to the wayward traveler to bridge the gap. They are willing to help though.

Hua Hin is quiet but here is plenty of action there, not as busy as your other well visited places but loads of girls and plenty bars.

About 2-3 hours by cab or minibus (check out your travel agents in BK) and by minibus cost me 800 Bht in Oct 2005 bus has a/c and sits about 8 persons if full. A taxi from the airport is about 2000-2500 to get there

The Casa Del Mare is a small, low-rise, fairly new 4* boutique hotel. The rooms are superb - kitted out to a very high standard - and the swimming pool is good but it isn't cleaned frequently enough. The rather small restaurant is quite poor, including the free buffet breakfast. Staff are mostly friendly and helpful. This hotel is more than a stroll to the town centre, but the busy, adjacent main road is well-serviced by taxis of the two and four wheeled variety. The hotel is not GF, but I would give it a conditional thumbs-up as a place to take a BKK, Pattaya, etc friend for a trip (based mainly on the rooms and staff).
The Sofitel had some bad press on a competing message board.
The Hilton has the best disco in town (The Hua Hin Brewing Company), the best beer in town, which, as the name suggests, is brewed on-site (C'mon Dom, don't be jealous toward those of us with healthy livers!), the best house band in town (check out their phenomenal version of Blur's 'Song 2'), and a fucking glut of freelancers after the beer bars of Soi Bintabaht across the road close at around 12 o'clock.
I have been to Hua Hin a couple of times. First time i stayed at the Hilton, and i found it to be nothing special, and the food not great, so i would not recomend it, but the pool area is ok.
I would suggest the Fresh Inn hotel, which is opposite the Hilton, is only around 1500 per night, and actullay very good, staff are friendly and helpfull. Romms are new, with all mod cons, and totally girl friendly. I have all the detials PM if you need the address.
When i stayed a few months ago, the pool was not finsihed but it should be by now, if not just walk accross the road and use the one at the Hilton.
There are a coupld of good resteraunts around there on that street, the italian opposite the Hilton is good, and theres a german bakery, which sell decent bread and does a good breakfast, which is a little but futher along the street.
As far aas the top end, it has to be the Chivasom, we went for lunch and it looks great.

I guess noon golf is to hot?? Therefore one needs to plan for the morning when most proper Bar staff are sleeping or still stupping, any strong leads or recommendations for a couple of highly trained proffesionals that can be hooked up in advance, ideally some bisexual ones.

But for a lot less money go for the private beach houses called Lanka Daeng (aka Railay Beach Club). Lived in those houses for several months.
Haven't been down there for years but Railay used to be very fucking cool.

I only played one round when I was in town, it was just too fuckin' hot.
The Royal course, at the back of the train station, is a hilly,well-maintained, mature (the oldest in Thailand, I believe) course. It's very convenient for the town centre. Green fee was about 1500bt, buggy and caddy a little extra. If the leather wedge is a shot in your repertoire, you will find yourself using it more than a coupla times on this track: All 18 holes are densely lined with mature trees.
