Once I arrived in Medellin, they checked neither.
In reference to an earlier post, the Scotiabank ATM I went to has the Colpatria branding. It's the one inside Jumbo La 70. This one charges a fee. They might have had started the fee recently since there was a report back from March that says Scotiabank wasn't charging fees.
The Davivienda on La 70 was closed these days but there's a sign on the door that pointed people to the one in Jumbo. This ATM doesn't appear on Google Maps when you search for "Davivienda ATM / Davivienda cajero" or whatever combination of that. It's at the back end of the Jumbo (not inside the supermarket but you need to walk along the entrance hallway down), a bit hidden away. This Davivienda also dispenses 50 mil bills vs the one on Cra 70 that dispenses 100's.

I was previously a registered user with them, meaning all of my Passport information etc. Was already in the system and when I filled out the form online last week to depart Colombia it asked me for my arrival information even though after I started the form checking the Departure box.
I tried several times and knew something was amiss, so decided to instead of logging in as a frequent user meaning all my Passport information I started the process without logging in as a frequent traveler and it went thru.
As far as USA on the menu, as I recall it's listed as Estados Unidos not USA.
Regardless, unless the airline asks for the form, I don't think it's going to be a problem if you haven't filled out the form beforehand as it saves time, although in Bogota they had people checking all arriving passengers and assisting them, but have never seen that in Medellin.

So when you give them your passport, they have all that information already entered and it makes their jobs easier.
