This taboo is probably cultural, which includes both heterosexual men and women. Such low prevalence of anal sex among heterosexual in Thailand is probably due to both men not wanting it and women not willing to do it.

Bangkok isn't exactly Isaan. In Isaan, female sex workers work almost exclusively with Thai men. While in Bangkok, millions of foreign tourists come every year from other countries.

It's possible that anal sex is more common among female sex workers in Bangkok than in Isaan. But I highly doubt that it's as common as you say. Because if what you say is true, then Bankgok massage parlours wouldn't be having so much trouble with recruiting A+ women to work for them.
