I'm planning a trip to Prague and got a couple of questions:

Just do the math, if it dips far below that it's a scam.

Well if the ATM is in a public space such as the main train station, Airport, the public transport hub, it is highly unlikely the "ATM is fake". I could not imagine how come a person put a fake ATM in the public space where the police, transport officials patrolling there have a trained eye to spot something unusual. But this is nothing to suggest they might not get tempered temporarily while you are using it. But my point here is that the ATM might get tempered by criminals but the ATM itself is genuine (not fake).

But what is likely happen if you are not careful is that you might get ripped off with exchange rates and various fees, etc. From certain ATMs. Unless you have tested it, avoid any ATM labelled with "EURONET". Not all card though, you will have to check what it is the to be applied to your card for foreign transaction. If you have this card this will be the best way of getting foreign currency from ATM. But in Prague money changer are notoriously know to fool tourists with fee and commission it is not just in "Jindriskka tram which is in the city centre, it is almost everywhere close to tourist spot actually.
