1) It's all about poverty and the extra bucks one could make for the next meal or bare necessities. In any case they are living in hell.

2) For top end, you can visit Alexis or Malioboro. At Alexis, I always preferred the sensual massage with FS with the Chinese girls in the 5th floor. All imports (EE, Thai, Viet and PRC) cost the same and pricing is segregated only between Local and Imports.
