For girls and sex, Hentai and Soapland have offices within walking distances of the Tower with English-speaking workers. The Japanese are well aware of the fact that Japanese is a difficult language to pick up. Most restaurants in popular areas have English menus, as well as menus that feature pictures of the food item along with their prices nearby. While many Japanese don't feel confident in speaking English, many of them understand it perfectly fine as long as you use basic words, speak at a reasonable pace, and don't use long run on sentences. Furthermore, the Japanese absolutely appreciate 'effort', if you simply attempt to speak a few Japanese words to get your point across, even if you stumble terribly, it will earn you a lot of goodwill and people will go out of their way to help you.

Upon entering an establish, I'd watch how people order and how they pay. Do you use a tablet? A ticket machine? Or are you waiting for a waiter or waitress to come by? When it comes time to pay, are people paying ahead of time? Waiting to ask for their check? Or are you given a slip that you take up front to pay once you're finished? Just knowing those basic details can help with avoiding awkward interactions if you don't speak Japanese. And remember, if you mess up, they won't be mad at you.

Some of the small izakayas will have Japanese only but you can use Google Translate camera to translate Japanese to English. If you still find things difficult, there's always McDonald's, Wendy's, Burger King, Denny's, etc.

Yamanote Line that's all you need to know. It circles Tokyo. You can't get lost. The massive train stations like Tokyo, Shinjuku, etc now that's another story altogether. Google maps is your best friend there. Even Japanese use it to find stuff.
