I went to Madhvi SPA (Blanca Paloma Spa) in Indiranagar. As one of the seniors suggested asked for Ivy. Asked for extras thrice during the massage session. She was like "What are extras?" and didn't budge till the end.

The Bangalore scene has been cricisized many times over here, including by yours truly.

You have hit the nail on the head. It is exactly the combination of many factors you have highlighted. The supply side problem is with low volume and poor quality exacerbated by cultural conditioning. On demand side we have way to many people with way too much money to spend. I have played in many top cities, but have observed a peculiar behaviour here in BLR high flyers to overpay, as if it gives an extra kick to them.
