To your first question. It depended on the agency. All the girls at THC brought it up first, either when I asked for the menu or during the session when things heated up. Last, it shows a coupon discounting 13K to 11K, with 1 set for 40 minutes. I bet if you went to Manzoku, they'd get you 11K or less. But note the pricing, it makes sense for an oppai pub. In fact, I remember paying 10K when I went 20 years ago. Japan has only had 10%-20% inflation in 20 years. Due to competition, you can usually tell what a shop does by its prices.
