Usually only the BDSM crowd are into it, and I'm too vanilla for their tastes. The roleplay was genuine. The owner has a second place called Canary Massage (Sukhumvit soi 19). Never been but the manager said they had similar roleplays. Looking at their websites the rooms show it. But he said that Psyche Dream was even more roleplay. At Canary it sounded like it was more like you describe at Kokoro. More just in the beginning. At Psyche Dream you shower yourself to start and the girl waits in her uniform. And then you get going. So more a proper roleplay.
Spa question: The onsen area is pretty quiet. I only saw two other guys there. I wouldn't sleep there (I guess you could take a nap) but you can easily chill or read there. Plenty of sun beds style chairs to lounge in. And you can stay as long as you want. The idea is to do the two massages after each other but you don't have to.
