It is an extra $82 to add additional pages. A new passport costs $110. Not much of a savings. As a pilot we don't bother with the extra pages. Often, if you ask at customs, they will not stamp your passport. Suprising how many countries will do that. I have only two re-entry stamps back into the USA and no entry stamps into EU.

[/QUOTE]A UK or International driving license is valid to drive in the DR for three months. After three months, a Dominican driving license is required. It can be requested to the "Oficina de Tránsito Terrestre", Ministry of Public Works & Communications.
Keep in mind your Tourist Visa (card) expires after 30 days so that would have to be renewed. Obtaining car insurance would be another hassle. Bottom line is don't do it.
