I often used to combine MDE and Sosua. One week in one place and then take a Copa flight to the other through Panama City for another week. That way you get one beach vacation and one city vacation.
If you have United / Star Alliance miles, it's frequently only 10 k miles from the DR to MDE (through PTY). If you really want to mix it up, stop in PTY for a night or two.

It depends on where you live, how much time you have, what interests you have outside of boning chicas, type of chicas you like, and so on.
