The easiest way is to go over time.

E. G. There is no real provision for a special fee for 45 mins. You are expected to pay for 100 for 1 hour.

So if you spent just 45 mins, consider 25 out of the 100 to be a tip, if you like.

I generally do not tip unless the service was exceptional. Tipping is a funtion of what was done and how much I have already spent. So, a thank you tip would be 5E-a bit more if she spent substantial time with me that she did not charge for. Otherwise, I spend more time in the room and that would be considered a tip.
