Most say no, but there are a few who will. First I should mention that it does require some ability in japanese. I'll go over things you need to know how to say below. Second, yes they serve to foreigners, and yes it is always fs.Third, the service is not always as consistent as the japanese counterparts. If it isn't already ridiculously obvious, most places photoshop the crap out of the pics on their websites - so don't expect the girl to be as ddg as she is on the site. There are a couple sites that don't, and it's worth checking them out to get an idea of what you can expect. I actually haven't encountered any B&S, but the pics can be old or photoshopped beyond recognition so that it seems like a B&S. Anyway, I'd say 80% of the places are based in Uguisudani. I think the cheapest hotels are around 3k - Hotel Elmer is pretty cheap if I remember correctly. You can go anywhere in tokyo really, even to your apartment but they charge an extra 3-5k and you have to get 120 minutes minimum (not bad if you can do multiples).
