i think you were deeply affected by the girl who left you to get married to someone else. i suggest you reassess your feelings calmly. are you acting out of some kind of anger towards her? she might have blamed you as a sex addict just as an excuse so that you don't blame her. at your age if you think about sex a lot, that is perfectly normal. yet you have managed to not have sex and yearn for a loving relation.

i also feel you are now self conscious about your looks / health.

my suggestion would be for you to focus on health first. that is the most important thing. go see a doctor. don't feel worried. these doctors see so many patients daily that they won't have the time to judge you, much less remember you and think about you.

join a gym / swimming class. you are still young and can still develop a great body. also these activities will keep you from thinking too much. in my experience thinking too much is always bad.

regarding guilt, it is personal. some people feel guilty for small mistakes and some don't feel guilty even after killing someone. do what you can live with.

all you need are some goals and time to work on them. set the ones that you believe will make you happy in the long run.
