A--A ayur spa it is. Don't remember the name of the therapist but you can choose from the selection.

Nothing. You may risk losing the cost of the hotel booking if she doesnot show up, if she does not have a ID proof, or if the hotel does not accept her ID proof.

For 8k you can get FS twice in Cali spa, thrice in any other spa if you are lucky, HJ for 4 times. Never go for that price my recommendation.
