Never had a girl check in at the front desk. Always I ask them to come to my room directly. Usually I stay on floor where you do not need key to access that floor.

Same. I prefer to not chek in the girl at reception. I usually book for 2 and tell them I may a have a friend joining later. 5 stars won't make any hassle.

But if you are meeting the girl for first time I would suggest go and pick her up from lobby without check in or see her in Lobby and then ask her to go to your room (if you feel awkward walking with her in lobby).

I was once set up by a trans pretending to be a young girl in Delhi SA (no offence meant to anyone). Good that I didn't give her the room no. And came to lobby to pick her up. Depends upon the hotel you live and the access to floors it has. Delhi you have many hotels where you can have the girl come up to you room without the access cards. But most of the 5 stars will need access for her to come up.

Its always safe that she goes to the reception and then come up or you go down and then bring her up.

SA I feel that is the best thing to do, Rest to each his own.

I have not got any issues so far, have tried couple of times in regular Oyos and townhouse. I assume that's how it works and mongering is an activity which always carries risk. I won't suggest to use a fake ID and get into bigger issues.
